Yes No Boarding Rules/Regulations - Your pet must be a current patient of Goose Creek Pet Hospital (current exam by one of our veterinarians within the past year). - You must have a reservation for your pet- we cannot guarantee openings, especially during peak boarding seasons. - Your pet must be current on vaccinations. We require your pet to have a current Rabies, DAPP, and Bordetella vaccine PRIOR to admission date. Exemptions for vaccines are determined by a veterinarian at Goose Creek Pet Hospital. - Your pet must be current of flea/tick and heartworm prevention. Goose Creek Pet Hospital will not be held liable if your pet contracts parasites during their stay. IF intestinal or skin parasites are found on your pet during their stay, they will be treated by veterinarian on staff. - If your pet is to be picked up by someone other than the name listed above, arrangements must be made with the boarding/daycare employees and the invoice balance must be paid prior to pick up. The name and phone number of the alternate pick up must be listed at drop off time. - Baths are scheduled at the owner’s request when the boarding reservation is made. Appropriate fees will be added to the invoice balance. If baths are requested after your pet has been dropped off, they may be unable to be completed during your pet’s stay. - All new dogs participating in doggy daycare must have a temperament evaluation by a daycare employee and/or a GCPH veterinarian. - All dogs participating in daycare must be spayed/neutered (NO EXCEPTIONS). - All reasonable precautions will be used to prevent injury and escape of your pet during it’s stay. Goose Creek Pet Hospital will not be liable for associated costs. * I have read and accept the rules and regulations put in place by GCPH If any treatments are needed during your pet’s stay: * Treat my pet as needed. Do all diagnostics and treatments deemed necessary for the well-being of my pet. I accept full financial responsibility for all charges related to the treatment of my pet. Treat my pet as needed, but do NOT exceed $200 without my consent. I understand if the proposed treatment exceeds the amount discussed and I cannot be contacted, my pet will receive appropriate care by the doctors at GCPH. I will be held responsible for all charges.
